Fees
Our fees are determined based upon the licensure status of
the therapist as well as the income level of the client or client’s parent(s).
For sessions with a licensed therapist, the cost is $125.00 per therapeutic
hour (50 minutes).
The fee for sessions with a Registered Intern is $80.00 per therapeutic
hour (50 minutes).
Registered Interns work under the direct supervision of Erin C. Falvey,
MFT. Additionally, there are a few sliding scale fee appointments
available which are based upon the income of the client or the client’s
parent(s). The
Program Supervisor will discuss this fee structure with you and work
with you to
determine a fee that meets your needs via phone or email prior to your
initial consultation
with your therapist.
Session fees are applied per session and must be paid for at
the time of service. Fees also accumulate if sessions are not properly canceled. We require a 24-hour notice for cancellations.

Payment Information
We offer several choices for payment of services. We accept
cash and checks in the office at the time of the appointment. Or, for your convenience
you can pre-pay your balance by credit card using online paypal services.
Cancellation Policy
Your
therapy hour has been reserved for you. Appointment cancellations must be made
at least 24 hours before the agreed-upon appointment time. Should you cancel
before this time there will be no fee added to your account. However,
cancellations received with less than 24 hours notice or “no-shows” will be charged a full session fee.
About Insurance
Reimbursement
If you have a health
insurance policy, it will usually provide some coverage for mental health
treatment. Many insurance companies reimburse between 50-80% of the session
fees for an "out-of-network" provider, which Erin
Falvey is. It will be your responsibility to fill out and file your health
insurance claim forms, so that you can be reimbursed for the fees you pay for
therapy services. We will provide you with whatever information we can
(diagnostic and procedure codes, National Provider Identifier (NPI) number,
etc.) to facilitate your receiving the benefits to which your insurance
entitles you. However, you (not your insurance company) are responsible for
full payment of the fee that we have agreed upon. Therefore, it is important
that you find out exactly what mental health services your insurance policy
covers. You should carefully read the section in your insurance coverage
booklet that describes mental health services. If you have questions, you should
call your plan administrator and inquire. Questions you may want to ask them
include:
1) What percentage of
my therapist’s fees are covered
by my out-of-network mental health
benefits?
3) Does my policy
cover the procedures described under
CPT Code 90806 (Individual therapy) and CPT
Code
90847 (Couples/Family Therapy)?
4) Is
pre-authorization required in order for me to be
reimbursed for fees for therapy? If so, what is
my
authorization number?
5) How many sessions
are authorized?
“Managed Health Care Plans”
such as HMOs and PPOs often require advance authorization before they will
provide reimbursement for mental health services. These plans are often
oriented towards a short-term treatment approach designed to resolve specific
problems that are interfering with one's usual level of functioning. They may
require you to seek additional approval for more therapy after a certain number
of sessions. In our experience, while quite a lot can be accomplished in
short-term therapy, many clients feel that more services are necessary after
insurance benefits expire. We are not members of any managed care programs, so
we will not be listed on the "Panel of Providers" for any such
program.
You should also be aware
that most insurance agreements require you to authorize us to provide a
clinical diagnosis, and sometimes additional clinical information such as a
treatment plan or summary, or in some cases, a copy of the entire record. This
information will become part of the insurance company files.
*If client discontinues therapy services prior to completing pre-purchased sessions full session fees will be applied to completed sessions and the remainder will be refunded to the client.
